Confidentiality

Protected Health Information (PHI) about you will not be disclosed without your knowledge and consent, except as otherwise authorized by law. A HIPPA Notice of Privacy Practices is available on our website. It is important to note that we may use and/or disclose your PHI without your consent or authorization for the following reasons:

  1. If it is determined that disclosure is necessary to avoid harm to you or another person or the public, or that a serious/imminent threat of physical violence exists.

  2. If a reasonable suspicion of child abuse or neglect, of elder abuse, or of dependent adult abuse.

  3. If legally compelled by federal, state, or local law; by judicial, board, or administrative proceedings; or by subpoena in a court proceeding.

  4. In a CCLS consultation group, your case may be discussed with other CCLS professionals, but your identifying PHI will not be disclosed.

Fee Schedule and Payment Policy

Please contact us for hourly fee schedule. The rate is prorated for extended sessions, phone calls, or third party consultations lasting more than 10 minutes and the preparation of reports, letters, etc.

Fees for contract services, consultation, and supervision are set on a case-by-case basis.

Payment is expected at the time of service. Cash, check and credit/debit accepted for payment.

Insurance Coverage

Services may be covered in full or in part by your health insurance or employee benefit plan. We are a self pay practice at time of service; however, if you would like to submit for insurance coverage we will provide you with a Super Bill upon request that you can submit to your insurance provider for potential reimbursement.

Cancellations

If you are unable to keep an appointment, for whatever reason, you must give 24 hours notice that you are canceling. Late cancellations (less than 24 hours notice) and no-shows will be charged as follows:

  • The first late cancellation, regardless of reason, will automatically be charged $75.

  • The second late cancellation will be charged $100.

  • The third and all subsequent late cancellations will be charged at the client’s full hourly rate.
